UTAH
STATE IMMUNIZATION REQUIREMENTS
FOR PRESCHOOL AGE CHILDREN
According the
Utah State Law, the following are the minimum immunization requirements
to be eligible to attend school in Utah.

1. 4
doses of DTP, DT, or DTaP vaccine
1st, 2nd, and 3rd doses should be at
least 1 month apart.
(The recommended interval is 2 months between the 1st, 2nd, and
3rd doses.)
3rd and 4th dose at least 4 months apart.
(The recommended interval is 6 months between these shots.)
2. 1 dose of Hib vaccine
Must be given on or after the first birthday
3. 3 doses of Polio (IPV or OPV)
1st, 2nd and 3rd doses should be at least 1 month apart
4. 1 dose of MMR
Must be given on
or after the first birthday
The Following Immunizations are Recommended Before Enrolling in Head
Start
2 doses of Hepatitis A
1st dose must be given on
or after the student’s second birthday
2nd dose must be given at least 6 months after the first dose
1 dose of Varicella
Must be given on or after the student’s first birthday
If student has a history of chickenpox, parent must sign
the Utah School Immunization Record stating the student has had
the chickenpox disease
- A school official cannot admit a student without proof of complete
immunizations, evidence that a student qualifies for conditional
enrollment, or evidence that a student qualifies for exemption on
medical, personal, or religious grounds. Forms for exemptions may
be obtained at the Health Department or for medical exemptions from
the child’s physician and must be kept on file for the child
at the school.
- Schools may conditionally enroll a student who is not completely
immunized against each specific disease if the student has received
at least one dose of each specific vaccine required and is as up-to-date
as possible. To remain in school, the student must complete the required
subsequent doses of each vaccine series on schedule and provide written
documentation to the school official.
- There is a 4-day grace period for all shots.
